Clinical Study of Surgical Repair Septal Perforation using Bipedicled Mucosal Advancement Flap with Temporal Fascia


Abstract
Nasal septal perforations are not common and difficult to manage.
The causes of nasal septal perforations are numerous and the most common one is previous septal surgery.
Five patients suffering from nasal septal perforations were repaired surgically utilizing bipedicled mucosal advancement flap of nasal septum and nasal floor with underlying temporal muscle fascia autograft from March 1993 to March 1994.
@ES The results were as follows.
@EN 1. The major symptoms were nasal obstruction, dryness, headache and whistling sound during respiration.
2. The causes of the nasal septal perforation were previous submucosal resection of nasal septum in three patients and the other causes were unknown.
3. Three patients had medium sized peforation and two patients had small-sized perforation Locations were cartilaginous septum in small sized perforation and bony-cartilage junction in medium sized perforations.
4. The concomitant pose and sinus diseases were two chronic paranasal sinusitis, two nasal septal deviation and one saddle nose. Combined endoscopic sinus surgery were performed in two patients.
5. Complete closure was shown in three patients, partial closure in one and reperforation in one to preoperative size after surgical repair.
A reperforated case was managed by septal silastic button obturator for closure of septum.
